Occasionally, you may need to leave your children with a sitter overnight or for a short vacation. MacNanny provides a medical consent form you can use which gives the caretaker permission to seek medical treatment for your child(ren) while you are away. You might also need a form for the day care center or preschool. For example, we have a form on file with our children's pediatrician giving the doctor permission to provide emergency care in our absence.
We strongly advise you to have your medical consent form reviewed by your physician, attorney, and hospital to determine its applicability and legality in the state of use.
Select the Medical Release item from the Options menu to display the medical Consent Report window (see Figure 15).

MacNanny will build a consent form for the first child in the Children's list. Check the information for accuracy and completeness before you print the medical consent form.
Selecting Notary Line prints a space for a notary public to notarize the form. Your doctor or attorney can advise you as to the requirements for a notarized form.
Once you have selected a child's name from the Consent Form list, you can click the Print icon or select the Print item from the File menu to print a copy. You can also change the font, style, and type size of the report.
Check the form for accuracy and completeness once is printed. Again, we strongly advise you to have the medical consent from reviewed by your physician, hospital, and attorney to determine its applicability and legality in the state of use.
